Applying for Change of Student Enrollment Information
1. Materials to prepare for a change of student enrollment information (including name, date of birth, or ID card number)
● Application form for change of information.
● Photocopy of the front and reverse of your ID.
● Original copy of your household registration, which must note what information has changed.
● A Change of Student Enrollment Information application fee of NT$10. Pay using the Automated Payment System, in the lobby on the first floor of the Educational Building.
2. Apply for a change of address or phone number (including contact, household registration, parent, or guardian address/phone number)
● Bring your ID card, if you are changing your household registration address. The card will be used to verify such a change.
● Go to the Registry Division, on the 3rd floor of the Educational Building, and apply for a change of address/phone number.
3. Procedure for changing your student enrollment information
● Make sure that your materials listed above are complete. Then, go to the Registry Division, on the 3rd floor of the Educational Building, and complete the change procedure.
4. Points of note when applying for a change in student enrollment information
● If your need to change campus cards, follow the Replacement of Campus Card procedure.
● If you are changing your name, notify your instructors in class of the change.
