1. Apply for transcripts in Chinese

● Transcripts in Chinese are NT$10 each. Pay using the Automated Payment System, in the lobby on the first floor of the Educational Building.

● After you complete payment, the machine will automatically print out your form(s).

2. Apply for Proof of Study

● A Proof of Study in Chinese is NT$10 per copy; a Proof of Study in English is NT$20 per copy. Pay using the Automated Payment System, in the lobby on the first floor of the Educational Building.

● To collect your Proof of Study, once you’ve paid as above, bring your receipt to the Registry Division, on the 3rd floor of the Educational Building. For a Proof of Study in English, you will need to provide your full name in English.

3. Replacement of Campus Card

● The administrative fee for replacing your Campus Card is NT$100. Pay using the Automated Payment System, in the lobby on the first floor of the Educational Building.

● Go to the Registry Division to register for a replacement Campus Card; give them your payment receipt to begin the process. While your card is being replaced, for cafeteria meals, you will need to enter your student number and PIN.

4. Documents required to apply for English transcripts, English graduation certificates, and English letters

● NT$20 per copy for English transcripts. Pay using the Automated Payment System, in the lobby on the first floor of the Educational Building.

● Once you have made your payment, bring the receipt to the Registry Division on the 3rd floor of the Educational Building and provide them with the following information:

▲ Your English name. The spelling you provide must match the spelling in your passport exactly. Alternately, you can provide the Registry Division with a copy of your passport.

▲ Your date of birth.

▲ Your Department and Section.

▲ Your place of birth.

▲ If you need your new card to be sent by mail, you must supply postage, and provide the recipient’s name and address. Delivery takes 7 working days.
